Background:The role of topical finasteride in treatment of benign prostatic hypertrophy (BPH) has been investigated in more than 100 consecutive men. While topical finasteride has been demonstrated to reduce the progression of BPH, it is not known whether topical finasteride is associated with clinical benefit, or if the benefit is due to adverse effects from the topical formulation. The purpose of this study was to investigate the effects of topical finasteride treatment in an Australian population.
Methods:We conducted a cohort study of 524 consecutive men aged ≥40 years diagnosed with BPH from 1995 to 2004. A multicentre randomised controlled study (RCT) was conducted to compare the effects of finasteride treatment in an Australian population to those of placebo. The men were randomised to receive either finasteride 1% topical solution or placebo. The men were assessed clinically on the PROSPERO scale, BPH symptoms, and the PROSTI scale. The PROSTI scale is a questionnaire that provides a visual analog scale (VAS) to indicate the severity of BPH symptoms and the frequency of BPH. The PROSPERO questionnaire is a validated questionnaire that assesses the frequency of urinary symptoms (e.g., urgency, pain, or difficulty urinating).
Results:A total of 524 men were included in the cohort study. The mean age was 49.2 years, and the majority of the men were female. Most men (n=232, 63.6%) reported having had prostate surgery within the past year. The mean duration of prostate surgery was 9.3 years, and most men (n=207, 74.1%) had a Gleason score of 4 or less at diagnosis. A significant reduction in prostate volume, urinary flow, and total prostate volume were noted in both the placebo and finasteride groups compared with the group receiving the placebo (P<0.001 for both).
Conclusion:Topical finasteride treatment has a statistically significant reduction in prostate volume and urinary flow, and can be an effective treatment option for men with BPH.
